Vendor Information Submission
First off, thank you. The Wisconsin Perfusion Society would not be able to put on such a great meeting for us to share our knowledge without your support.
Please fill out the following information to facilitate some of our activities at this years meeting.
How this information will be used:
WPS Website:
Registered vendor logo, company name, and linked website will be included on the Wisconsin Perfusion Society Website for 12 months from April 2026 to April 2027.
Vendor Raffle:
Each registered vendor will receive raffle tickets to hand out at their booth to attendees. These tickets will tear apart for raffle entry. The "keep this portion" of the ticket will contain your company logo and local representative contact information to serve as a "business card." Vendors have the opportunity to donate raffle prizes, otherwise prizes will be sponsored by WPS.
Local Representative Contact Sheet:
WPS keeps a directory of our local vendor contact information. This is available upon request by members. Emails and phone numbers will not be publicly posted on the website in order to discourage spam.
Acknowledgement for Sponsored Activities:
We would love to use your logo for signage to recognize you during any sponsored meals or activities!
